In which order does swimmers swim in Olympic Swimming medley?

Backstroke, Breaststroke, Butterfly and Freestyle

Olympic Swimming medley is a combination of four different swimming styles —Butterfly, Backstroke, Breaststroke, and Freestyle— into one race. This race is either swum by one swimmer as individual medley (IM) or by four swimmers as a medley relay.
Back goes first because in the rules for backstroke you can only turn past vertical to do a flip turn.

If a swimmer breaks, for instance, the 100-meter free stroke record in a medley event does that record count towards the hundred meters discipline or are those records held apart?

I'm curios why a bronze medal is not awarded in 100m butterfly swimming Rio 2016 award ceremony. Michael Phelps and two other swimmers got silvers but no bronze was awarded.
